Program Description

The Personalized Community Care (PCC) Program is designed to ensure youth who have experienced crisis have access to a safe, welcoming, and highly supportive home environment. The program is grounded in trauma-informed practice and draws upon each individual’s unique strengths, needs, and vulnerabilities to foster meaningful relationships and support the development of healthy emotional regulation, problem-solving skills, and positive ways of responding to stress and challenge.

Typically, youth are referred through Children and Family Services either on an emergent basis or due to higher or more complex needs, often including multiple diagnoses and involvement in high-risk circumstances. The PCC Program is intentionally structured to promote stability, predictability, and a strong sense of safety and security, creating the conditions necessary for healing, growth, and long-term progress.

This program delivers services across the North Region of Alberta and the Edmonton area.
