Program Description
The Personalized Community Care (PCC) program is designed to ensure youth who have experienced crisis have a safe, welcoming home. This program focuses on a trauma-informed approach and utilizes the unique combinations of the individual’s strengths and vulnerabilities to promote relationship building and utilization of coping strategies.
Typically, youth are referred through Children and Family Services either on an emergent basis or due to having higher needs (often with multiple diagnosis and living an "at-risk" lifestyle). The PCC program aims to create a sense of stability, predictability, and security.
