Program Description

The Foundations Transitional Living Program is a residential support program designed to bridge the gap between child and youth services and adult service systems for young people who are in transition after 18 years old. This program specifically supports youth who are awaiting formal adult service approvals.

Foundations offers a balanced, developmentally appropriate approach that promotes autonomy, independence, and the development of adult life skills, while recognizing that young people in transition continue to require meaningful supports, structure, and guidance. Services are individualized and responsive, supporting each young person as they navigate increased responsibility alongside ongoing care.
